PATH (Projects for Assistance in the Transition from Homelessness)
Outreach and case management for adults living with a severe/persistent mental illness and experiencing imminent or literal homelessness

RHY-SO (Runaway and Homeless Youth Street Outreach)
Outreach services for youth 21 and under focused on moving youth into stable housing and to prevent the sexual abuse or exploitation of young people living on the streets or in unstable housing.

The Youth Homelessness Demonstration Project (YHDP) System Navigation program connects young adults ages 18–24 who are at risk of or experiencing homelessness with housing and supportive resources.
